Intake, consent & agreement forms

Build intake and consent forms clients fill out and sign on their phone before a visit, or a service agreement they sign when approving a quote. Signed copies stay on the client record.

Get the paperwork done before they arrive

Allergies, health history, a waiver, a work authorization: collect it ahead of time instead of on a clipboard at the front desk. Clients fill out your forms on their phone and sign with a finger, and the signed copy lands on their client record.

Set up a form

Go to Business → Forms. Start from a template (Health intake, Consent & waiver, Massage preferences, or, in Jobs mode, Service agreement) or a blank form, then make it yours:

  • Intro or agreement text: a short note, or your full terms
  • Questions: short answer, paragraph, yes/no, checkbox, or multiple choice, each optional or required, in any order
  • Signature: on by default. Clients type their full name and draw their signature
  • Which services: all of them, or only the ones you pick
  • How often: once per client (a health history) or every visit (today’s preferences)

Each form has an on/off switch, so you can pause one without losing it.

How clients get it

When a booking needs a form, the confirmation text says so and links to the client’s manage page, where it waits under Before your visit. The reminder text asks again if it’s still not done. Once it’s signed, it shows Done and the texts go back to normal.

What you see

  • On the appointment: each form shows Completed or Not completed. Tap a completed one to read the answers and signature. Copy link gives you the client’s link to send yourself.
  • On the client: Forms on file lists everything they’ve signed, newest first.
  • As it happens: you get a notification the moment a client submits.

Agreements on quotes (Jobs mode)

In Jobs & Field Service mode, a form can be used for Quotes instead of visits. Turn on Add to every quote and clients read and sign it on the quote page as step one, then approve (and pay a deposit, if you take one). You can pick a different agreement, or none, on any job under Agreement. The job shows Signed with who signed and when.

Signed copies don’t change

Each submission keeps an exact copy of the form as it was signed, with the date and time. Edit a form later and past signatures still show what the client agreed to. Removing a form stops sending it; signed copies stay on your clients’ records.

Forms work in both appointment and Jobs mode. Agreements on quotes are part of Jobs & Field Service, which requires Premium.
